Presentation

Right iliac fossa pain

Patient Data

Age: 45 years
Gender: Female

A well-circumscribed hypodense lesion with peripheral rim calcification and internal septation was observed in the right iliac fossa, likely originating from the appendix, measuring 5.7 x 4.9 x 6.4 cm (AP x W x CC). No solid component was detected within. There was no definite enhancement or surrounding fat streakiness. Additionally, no enlarged regional mesenteric lymphadenopathy was noted.

There were no other bowel-related masses or abnormal bowel dilatation. Focal liver lesions suggestive of metastasis were not present. Cholelithiasis was noted.

Case Discussion

Features are suggestive of low-grade appendiceal mucinous cystadenoma. The differential diagnosis includes high-grade appendiceal mucinous cystadenocarcinoma or appendiceal mucocele. Histopathological analysis is crucial to obtain the correct diagnosis.

The patient underwent a right hemicolectomy and removal of the right adnexal cyst. The patient was discharged well.

Histopathological examination reveals:

  • right hemicolectomy: low-grade appendiceal mucinous neoplasm (LAMN)

  • right fimbrial cyst: benign paratubal cyst
