Cesarean section scar diverticulums are a defect in the lower uterine cavity at the site of the cesarean section scar. 

Clinical presentation

  • mostly asymptomatic
  • postmenstrual spotting
  • in a study was found to be the only finding in patients with bleeding disturbances

Radiographic features

Can be imaged on hysterosalpingography and ultrasonography.

There may be a defect defect or a focal outpouching at the site of prior cesarean section. Location can be in the lower uterine segment or upper endocervical canal.

Differential diagnosis

  • prominent cervical glands 
  • post myomectomy diverticula 
  • Gartner's duct cyst: long, tubular structure parallel to uterine cavity
  • adenomyosis: location of defect anywhere along uterine cavity
