If the RCAright coronary artery (RCA) has an aberrant origin from the the left coronary sinus, it can have an inter-arterial course where it passes to the right between the ascending aorta and the pulmonary trunk. It is a rare variant of RCA anatomy.

It is described as malignant as the vessel can be extrinsically compressed by the two large arterial structures, leading to RCA territory myocardial ischemia, myocardial infarction or sudden cardiac death especially in the athletic young population.

The orifice is often 'slit' like, contributing due to the risk of cardiac events from impairedit's oblique orientation which can also contribute to  impaired RCA blood flow.
