Sacroiliitis grading can be achieved using plain radiographs according to the New York criteria41,2.

Classification

  • grade 0: normal
  • grade I: some suspicious changes (some blurring of the joint margins - suspicious)
  • grade II: minimal sclerosis minimum abnormality (small localized areas with some erosionerosion or sclerosis, with no alteration in the joint width)
  • grade III
    • definite sclerosis on both sides: unequivocal abnormality (moderate or advanced sacroiliitis with erosions, evidence of joint 5
    • severe erosions withsclerosis, widening of joint space with, narrowing, or withoutpartial ankylosis
    )
  • grade IV: complete severe abnormality (complete ankylosis)
