Tibial tuberosity to posterior cruciate ligament distance (TT-PCL distance)

The tibial tuberosity to posterior cruciate ligament (TT-PCL) distance is an alternative measure to the TT-TG distance for determining tibial tuberosity lateralization, which has a role in patellofemoral instability, without the effect of femoral rotation 1.

TT-PCL and TT-TG have similar reliability and sensitivity/specificity, however, TT-TG has greater diagnostic accuracy 5,6.

The TT-PCL distance is measured on axial MRI between the patellar tendon insertion midpoint and the medial border of the posterior cruciate ligament at its tibial insertion, parallel to the dorsal tibia condylar line 2,3.

  • normal range: 11.9 ± 4.7 mm 2,3

  • >20-24 mm: patellofemoral instability 3,4
