Presentation
Abdominal pain. Recurrent UTIs. Stone? Elevated CRP and neutrophils.
Patient Data
7x6 cm thick-walled cystic lesion with surrounding inflammatory changes anterosuperior to the dome of the bladder in the midline of the pelvis.
No free fluid or collection.
Normal upper abdomen.
7x6 cm thick-walled cystic lesion with avid internal diffusion restriction anterosuperior to the dome of the bladder in the midline of the pelvis.
No superior midline tract.
No free fluid.
Case Discussion
The appearances are consistent with an infected urachal cyst.
As is typically the case, the cyst has been present since birth but becomes apparent when it becomes infected as demonstrated by the internal restriction on MRI and surrounding inflammatory change in the CT.
