Undisplaced intertrochanteric fracture appears as a low signal serpiginous line on T1 and a high signal line on the fat saturation images.

Case Discussion
Fractures of the proximal femur may not be visible on radiographs or even CT scans in demineralized patients. MRI is much more sensitive for fracture and should be strongly considered in elderly patients who fall with hip pain and inability to bear weight.
