Presentation

Cough. No fever.

Patient Data

Age: 90 years
Gender: Male

PA chest radiograph reveals a mediastinal mass, which is in continuity with the left heart border. The hilum is visible through the mass.

A CT study revealed this to be a loculated pleural effusion.

Case Discussion

There is a wide differential for mediastinal masses.

Loculated pleural effusions can occur in isolation and, as in this case, be idiopathic.
