Presentation
Cyanosis since birth. Status-post thoracic surgery. CT scan was requested for preoperative evaluation.
Patient Data
CT demonstrates the right-sided aorta arising from the right ventricle and the main pulmonary artery arising from the left ventricle consistent with D loop transposition of the great arteries. A ventricular septal defect is seen. There is a surgically created connection between the left-sided brachiocephalic artery and left pulmonary artery. Sternotomy wires are noted.
Case Discussion
This is a diagnosed case of D loop transposition of the great arteries. Patient previously had pulmonary valve stenosis and underwent systemic-to-pulmonary artery shunting (modified Blalock-Thomas-Taussig shunt) and balloon angioplasty to improve the pulmonary artery circulation prior to contemplated arterial switch procedure.
