Presentation

Back pain and sciatica.

Patient Data

Age: 80 years
Gender: Male

Lumbar spine

Spondylodegenerative changes manifested as decreased disc height and hydration signal with marginal osteophytes.

L4-5 posterior diffuse disc bulge indenting the thecal sac and encroaching on exiting foramina bilaterally and both lateral recesses more on the left side which shows a small 7 mm left facet joint synovial cyst, also noted hypertrophied ligamentum flavum.

L5-S1 posterior diffuse disc bulge indenting the thecal sac and encroaching on exiting foramina bilaterally.

Case Discussion

Radiculopathies are commonly due to disc lesions. One of the other lesions with similar effects is the spinal synovial cyst which arises from the facet joint, at the lumbar spine, which usually shows associated fact joint arthropathy. There is a predilection towards the L4-5 level.
