Pleomorphic adenoma - parotid gland

Diagnosis probable

Presentation

Progressive painless swelling of the right parotid gland of 5 years duration.

Patient Data

Age: 25 years
Gender: Female
ultrasound

The ultrasound images demonstrate a well-defined lobulated oval hypoechoic soft tissue mass with posterior acoustic enhancement within the superficial lobe of the right parotid gland.

The previously described soft tissue mass appears well-circumscribed of low signal intensity on T1WI, high signal intensity on T2WI with progressive heterogeneous enhancement on postcontrast dynamic sequence. A peripheral hypointense rim is noted on T2WI sequences representing most likely the fibrous capsule. No restricted diffusion seen on DWI/ADC.

The ADC within the mass is 1.85 x normal (>1.2 usually suggestive of pleomorphic adenoma).

The TIC (time signal intensity curve) shows a progressive and gradual enhancement of the tumor with delayed peak time at 213 s (TIC type A according to Yabuuchi et al classification).
