Subarachnoid hemorrhage

Presentation

Left-sided weakness with a history of head trauma 12 days ago.

Patient Data

Age: 90 years
Gender: Female

Subacute stage subarachnoid hemorrhage is seen in the right central sulcus, demonstrated as hyperintense on T1 images, blooming artifact on SWI images and hyperintense on filtered phase images (note, the same intracranial vessels). Findings are consistent with late subacute stage of hemorrhage.

Ex-vacuo dilatation of the ventricles due to age-related brain parenchymal atrophy.

Chronic ischemic changes are also seen with old lacunar infarcts.

Case Discussion

Subarachnoid hemorrhage is a type of extra-axial intracranial hemorrhage, demonstrating blood within the subarachnoid space. Although MRI is thought to be more sensitive, CT is frequently performed first due to wider availability. As well as being more sensitive for hemorrhage, MRI has greater sensitivity to the wide range of causative lesions.
