Area postrema syndrome

Diagnosis almost certain

Presentation

Intractable hiccups and vomiting, since 15 days.

Patient Data

Age: 30 years
Gender: Female

A small focal non-enhancing T2 and FLAIR hyperintensity is noted in the dorsal medulla oblongata, at the floor of the fourth ventricle without any mass effect.

x-ray

Chest x-ray is unremarkable. No focal mediastinal lesion.

Case Discussion

The patient's hematology, biochemistry and autoimmune workup, abdominal imaging and upper endoscopy were all normal. MRI brain features and patient’s intractable hiccups, suggestive of area postrema syndrome, usually seen in the context of neuromyelitis optical spectrum disorder. The patient completely recovered following treatment with steroids.
