Patient Data

Age: 35 years
Gender: Male

The medial part of the proximal calf namely the medial gastrocnemius muscle is the seat of a large heterogeneous mass measures about 4x6.5x15 cm at its largest ap, tr, and cc dimensions respectively. In T1-weighted images it shows an increase in the fat content with multiple interlacing vascular channels in the lesion, giving a “bag of worms” appearance. On T2-weighted images, the mass shows a high signal intensity of intralesional fat that is suppressed in T2FS and STIR. STIR images show the vascular component of the lesion to be markedly hyperintense to the muscle.

CT study revealed a fat-containing mass with small spherical calcifications characteristic of phleboliths.
