Superior peroneal retinaculum injury with peroneus brevis subluxation
Presentation
Three weeks post inversion injury with difficulty to weight-bear.
Patient Data
Anterolateral cutaneous edema around the ankle joint.
Anterior inferior tibiofibular ligament is intact but high signal. Posterior inferior tibiofibular ligament is intact. Anterior talofibular and calcaneofibular ligament are intact. High signal of the anterior deltoid ligament. Superomedial spring ligament appears thickened.
Dorsal talonavicular ligament is thickened with dorsal navicular bony spurring. Bifurcate ligament is thickened with bony spurring at the anterior calcaneal process suggestive of prior injury.
Superior peroneal retinacular injury with subluxation of peroneal brevis to lie lateral to the lateral malleolus, peroneus longus remains posterior. Mild common peroneal tendon sheath thickening and high signal with small effusion of the peroneus longus tendon sheath. Incidental peroneus quartus. Flexor and extensor tendons are intact.
Case Discussion
Superior peroneal retinaculum injuries are uncommon, here peroneus previous tendon is subluxed whereas peroneus longus tendon is not. In addition, there is a partial tear of the anterior inferior tibiofibular ligament representing a syndesmotic injury with findings of a prior midtarsal injury.
