Small bowel obstruction - indirect inguinal hernia

Presentation

Abdominal pain, nausea, vomiting.

Patient Data

Age: 60 years
Gender: Male

Dilation of small bowel with a transition point in a right inguinal hernia lateral to the inferior epigastric artery containing a short segment of mildly thickened small bowel. The distal small bowel is decompressed.

Large hiatus hernia. Cholelithiasis. Uncomplicated colonic diverticular disease. Enlarged prostate.

Case Discussion

Small bowel obstruction due to an incarcerated indirect right inguinal hernia. Often surgery will attempt to reduce this manually before proceeding to surgical repair. It is helpful to review cases like this on both axial and coronal images and trace out the bowel to improve your understanding of the anatomy.
