The patient's mother reported a history of sore throat and rhinorrhearhinorrhoea in the past few days. Multiple T2/FLAIR hypersignal white matter lesions with the appropriate clinical history led to the diagnosis of ADEM.

There are multiple indistinct regions of T2/FLAIR high signal intensity in the subcortical white matter, bilateral internal capsule, left thalamus and brainstem. The lesions are imperceptible in T1W images and do not show restricted diffusion in DWI/ADC images.
