Bilateral fornix acute infarction

Case contributed by Thành Anh Lê
Diagnosis certain

Presentation

Amnesia, irrelevant talk for 3 days.

Patient Data

Age: 40 years

There is prominent diffusion restriction at the bilateral anterior fornix and in the small nuclei of the right thalamus (possibly the anteroventral nuclei).

Normal MRA with normal ACOM

Case Discussion

The bilateral columns of the fornix are supplied by the subcallosal artery, which mostly originates from the anterior communicating artery (which appears normal in this case).

Additionally, the subcallosal artery also supplies the genu of the corpus callosum and other basal forebrain structures. In this case, the genu of the corpus callosum appears normal. However, it's common to see infarction there in many case reports.
