Presentation
HIV+, worsening abdominal pain.
Patient Data
Multiple lung nodules of varying sizes in the imaged lung bases.
Few round, intermediate attenuation hepatic lesions. Heterogeneous pancreas with a few ill-defined lesions. Several rounded right renal cortical lesions. Extensive infiltration of the left kidney and left perinephric space.
Extensive retroperitoneal and mesenteric adenopathy. Extensive soft tissue infiltration of several small bowel loops. Nodular soft tissue infiltration of the descending colon and left paracolic gutter.
Case Discussion
AIDS-related lymphomas are highly aggressive neoplasms which often present with atypical features and extranodal disease, with a poor prognosis 1.
As in this case, patients have higher incidence of splenic and hepatic involvement, increased mesenteric adenopathy, bowel involvement, and renal lesions. Also noteworthy in this case is the infiltration of the pancreas.
