Presentation
Recurrent dislocation of the right shoulder
Patient Data
Roughness of anterior-inferior glenoid labrum.
Irregular antero-inferior labrum with fracture of the corresponding bony glenoid.
A bony defect is noted at the postero-superior aspect of the humeral head.
Moderate joint effusion.
CT confirms the presence of bony Bankart and Hill Sachs lesions.
Case Discussion
Hill Sachs deformity is a posterolateral humeral head impaction fracture due to impaction against the antero-inferior labrum in the setting of antero-inferior dislocation, which is the commonest type of shoulder dislocation. This is associated with a Bankart lesion, which is a cartilaginous and/or bony defect of the antero-inferior labrum.
