Choledochal cyst - type Ia

Patient Data

Age: 5 years
Gender: Female

Saccular dilatation of mid common bile duct directed superolaterally with calculi in its dependant part.

Anomalous, high pancreaticobiliary junction is seen with a common channel measuring approx 8.7 mm draining into major papilla. Distal CBD is stenotic just before joining the common channel.

Distal CBD above the level of stenosis is dilated, measuring approx 8 mm in maximum diameter with calculus measuring approx 6.5 mm within.

Cystic duct is dilated, measuring approx 8.5 mm in maximum diameter. It is possibly draining into above-mentioned saccular dilatation of mid common bile duct. Common hepatic duct is also dilated, measuring approx 10.5 mm in maximum diameter.

No significant dilatation of intrahepatic biliary radicals is seen.

Gallbladder is distended with no obvious evidence of intraluminal calculi, wall thickening or pericholecystic edema.

Case Discussion

Overall above findings suggest likelihood of choledochal cyst, possibly type Ia with calculi within the cyst as well as in dilated segment of distal CBD.
