Contrast agent pooling sign

Contrast-enhanced CT scan shows IV contrast agent reflux into the inferior vena cava, intrahepatic veins, and right renal vein. (contrast agent pooling sign)
Other findings are small aorta, comminuted burst fracture of L1 vertebra, bilateral pulmonary contusions, rib fractures and right pneumothorax.
Bilateral intercostal catheters and endotracheal tube are seen.
Case Discussion
Inferior vena caval contrast reflux in this patient is due to massive pulmonary contusion, hypovolemic shock and right ventricular dysfunction.
