Ectopic testis seminoma

Presentation

Palpable mass in the left anterior abdominal wall

Patient Data

Age: 30 years
Gender: Male

Radiological findings include mass in the subcutaneous fat of the left anterior abdominal wall connected with spermal cord measuring 7.5 cm in the craniocaudal dimension. It shows abnormal signal intensity-restricted diffusion, heterogeneous contrast enhancement. MRI findings are compatible with anterior abdominal wall ectopic testicular seminoma. A biopsy confirmed the diagnosis.

Regional lymphadenopathy.

Additional findings: The scrotal sac is empty. The right testicle could not be identified. There is a 3.5 cm ejaculatory duct cyst.

Case Discussion

Ectopic testis refers to the testis entering a location other than the scrotum after passing the inguinal canal. It usually derives from overdevelopment and lengthening of a segment of the gubernaculum or from obstruction of the entrance to the scrotum.

Abdominal wall testis is the rarest form of ectopia.

Histology

Histologic evaluation showed a germ cell tumor with morphology compatible with a seminoma.
