Acute onset of gaze palsy, diplopia, mouth deviation, ataxic gait, headache with persistent vomiting. Known history of diabetes mellitus and hypertension.

A small recent infarct is seen involving the right abducens nerve nucleus and the facial colliculus, where the fibers of the facial nerve arch behind the nucleus of the abducens nerve.
Case Discussion
The patient presented with facial colliculus syndrome. The MR shows a small recent infarct involving the abducens nerve nucleus and the facial colliculus, correlating with the patient's symptoms.
