Left renal pelvis obstructing calculus containing multiple hypodense foci, most likely representing gas. Additionally, multiple small hypodense foci, again likely gas, are also demonstrated within the left renal pelvis and upper ureter.
Gas-containing urinary stones are very rare and are associated with anaerobic fermenting organisms such as Escherichia coli and Klebsiella pneumoniae. Risk factors include diabetes mellitus, gout, hyperparathyroidism, and sarcoidosis.
