Iliopsoas hematoma

Presentation

Complaining of right-sided back pain, right lower abdominal pain, and an inability to extend his right hip joint for the last two days.

Patient Data

Age: 25 years
Gender: Male

Referred for USG of A/P...

ultrasound

Referred for USG of A/P with suspected renal colic.

There is a diffuse enlargement of the right iliacus and psoas muscles. A collection with echoes is present involving the right iliacus muscle measuring 70 x 25 x 15 mm. The right psoas muscle shows 2-3 smaller collections  (10-15 mm) in the lower abdomen. There is a fluid rim along the anterior aspect of the right psoas muscle in its cranial end and along the posterior aspect of the right psoas muscle in the right iliac fossa region.

There were no renal calculus/backpressure changes on the right side.

Case Discussion

Findings suggest an iliopsoas hematoma in a patient with a known history of a bleeding disorder.

History of continuous back pain with inability to extend the right hip joint along with the absence of hematuria/dysuria were against suspected renal colic.
