Age: 10 years
Gender: Female

An intraventricular cyst is noted in the occipital horn, atrium, and temporal horn of the right lateral ventricle resulting in their dilatation with left side shift of septum pellucidum. The thin medial wall of the cyst is well-seen. The anterior horn of the right lateral ventricle is not dilated.

MRI is helpful for further confirmation.

A displaced shunt is seen in the right skull soft tissues.

Remodeling of the skull bone is noted likely secondary to shunt placement.

Case Discussion

Considering the mass effect of the cyst it is more likely an intra-ventricular cyst rather than internal porencephaly. An intraventricular simple cyst is a real space-occupying lesion while porencephaly is encephalomalacia that communicates with the ventricular system and/or the subarachnoid space
