Medial collateral ligament avulsion injury

Case contributed by Bahman Rasuli
Diagnosis almost certain

Presentation

Knee pain after motor vehicle collision

Patient Data

Age: 35 years
Gender: Female

Mild joint effusion is seen.

Bone marrow edema/bruise is present at the lateral femoral condyle, tibial plateau and fibula head related to impaction fractures.

Evidence of incomplete ACL tearing and reduced PCL angle due to buckling are seen.

Avulsion fracture of the femoral attachment of the medial collateral ligament is noted.

Subcutaneous fluid collection (hematoma) also is seen superficial to gastrocnemius muscle.

Case Discussion

Avulsion fracture at the femoral origin of the MCL is uncommonly associated with the bony fragment arising from the posterior aspect of the medial femoral condyle and irregularity of donor site in the background of valgus stress injuries.
