Near-drowning pulmonary edema

Presentation

Surfer found on the beach.

Patient Data

Age: 35 years
Gender: Male

Patchy airspace opacities throughout both lungs mid and upper zones, relatively sparing the apices. Retrocardiac left lower lobe area of consolidation. No pleural effusion is seen.

Patchy consolidative changes in both lower lobes associated with centrilobular nodules, highly suggestive of aspiration. Centrilobular groundglass opacities foci having a perihilar (mid and upper zone) distribution, suggestive of non-cardiogenic pulmonary edema. 

Case Discussion

This case shows features of lower lobes aspiration and non-cardiogenic pulmonary edema that are consistent with the presumed near-drowning incident.
