Osteitis condensans ilii

Diagnosis almost certain

Presentation

Right hip pain that now locks, clicks and gives way. Normal body mass index. No previous trauma. ?osteoarthritis

Patient Data

Age: 45 years
Gender: Female
x-ray

Marked bilateral iliac-sided subchondral sclerosis of the sacroiliac joints. Joint spaces appear preserved, with no erosive features.

Imaging features most likely represent osteitis condensans ilii, however, the differential would include sacroiliitis.

Marked subchondral sclerosis of the right acetabular roof, which may be related to degenerative change. Hip joints are well preserved. Bilateral coxa valga. Normal symphysis pubis. Sterilization clips noted.

Case Discussion

The patient has given birth before, which is thought to be associated with osteitis condensans ilii - likely due to mechanical stress and imbalance. Differential diagnosis would include sacroiliitis. In this case, this was felt less likely given the sacroiliac joint spaces persevered with no erosions or irregularity.

This patient has bilateral coxa valga, where the femoral angle of inclination exceeds >140 degrees.
