Osteochondral injury of the patella after patellar dislocation

Presentation

Football player sustained a non-contact twisting injury of the lower extremity with the knee extended and external rotation of the foot and traumatic lateral patellar dislocation with spontaneous patellar relocates.

Patient Data

Age: 20 years
Gender: Male

Presence of femoral trochlea dysplasia with hypoplasia of the medial femoral condyle (grade C according to Dejour class). Axial T2 images showing a large chondral injury to the patella articular surface with large loose body and abundant joint effusion. The patella is laterally subluxated and there is a small area of medullary edema of the external femoral condyle. The cruciate ligaments, the collaterals, the menisci and the medial patellofemoral ligament (MPFL) are normal.

Case Discussion

Osteochondral fractures can occur in acute or recurrent transient lateral patellar dislocation. The most common sites include the inferomedial patella or the lateral femoral condyle. There is an association between patellofemoral cartilage damage and patellar maltracking. The patella will often self-reduce by reflex contraction of the quadriceps muscles.
