Bilateral ovarian lesions containing fat, calcifationcalcification and solid soft tissue density elements - typical findings for ovarian dermoids.  The The larger left sided lesion contains intra lesionalintralesional fluid and there is free fluid in the pelvis.  In In the context of acute pelvic pain, left ovarian torsion is the most likely diagnosis.
