Proximal iliotibial band injury

Presentation

Onset of intense pain in the left iliac region in a marathon runner with a history of chronic lateral hip pain. Also, wwelling and ecchymosis from the iliac crest to the greater trochanter, with palpable tenderness over the affected region.

Patient Data

Age: 60 years
Gender: Female

The STIR sequence shows a focal area of fiber disruption/tearing of the proximal iliotibial band (ITB), anteriorly, with perilesional fluid collection extending caudally along the outer edge of the vastus lateralis muscle in addition, there is adjacent soft tissue and gluteus medius muscle belly edema. The hip joint and the iliac fascia lata attachments are normal.

Case Discussion

Degenerative lesions of the fascia lata without trauma can result from relatively minor physical activities and tend to occur in middle-aged and older women. As with traumatic injuries, hyperintensity surrounds the affected components of the fascia lata, with fiber discontinuity representing tearing.
