Shiny corner sign - ankylosing spondylitis

Presentation

Low back pain with right limb sensory change.

Patient Data

Age: 45 years
Gender: Female

The L5-S1 level shows the "shiny corner sign" of the anterior endplates - focal high T1 signal and high T2 signal with fat suppression on the STIR sequence.

Mild disc and mild-to-moderate facet degeneration with ligamentum flavum thickening. Bilateral L4-5 posterior juxta facet cysts. Mild spinal canal stenoses at the L1-2 to L3-4 levels. Mild neural exit foraminal stenoses at the L2-3 and L3-4 levels.

Case Discussion

The shiny corner sign is a reactive sclerosis to endplate inflammation, which in this case has fat-intensity signal in keeping with post-inflammatory degeneration. This is in keeping with the patient's long-standing history of ankylosing spondylitis and Crohn disease on biological treatment. No cause for the patient's presentation was identified on this study.
