Presentation

Chronic right hip pain

Patient Data

Age: 30 years
Gender: Male

Subchondral cystic changes of the anterior superior femoral head aspect with prominent tendon-like low T1/T2 signal intensity extending from the right anterior inferior iliac spine at the origin of the direct (straight) head of rectus femoris.

x-ray

X-ray shows a large bony spur extending inferior from the right anterior inferior iliac spine. Contact with the greater tuberosity during the frog-leg view.

Case Discussion

Subspine impingement is a rare type of hip impingement. In this case, it appears secondary to heterotopic ossification of the origin of the rectus femoris direct (straight) head, presumably secondary to prior injury.
