Aortic transection

Presentation

Motor vehicle accident with positive head strike and anticoagulation. Patient complaining of severe chest pain and diaphoresis.

Patient Data

Age: 80 years
Gender: Male

Contained perforation of the aortic arch posterior to the origin of the left subclavian artery. There is a large mediastinal collection of blood that results in lateral displacement of the esophagus and trachea to the right.

The left vertebral artery is not visualized.

Focal dissection with a displaced intimal flap along the right lateral aspect of the distal descending thoracic aorta without a periaortic hematoma.

Intraparenchymal hematoma within hepatic segment 4A.

Minimally displaced fractures of the bilateral ribs 4-7.

Case Discussion

This is a case of an aortic transection with pseudoaneurysm formation following a motor vehicle accident. The patient was emergently brought to the operating room by vascular surgery. He underwent placement of an endograft in the aortic arch. There were no post-operative complications.
