Presentation

Left breast pain. Breast augmentation 20 years ago.

Patient Data

Age: 60 years
Gender: Female

The right implant is intact with no features of an extra or intracapsular rupture.

The left breast implant demonstrates features of an intracapsular rupture with a subcapsular line sign and a linguini sign inferomedially. There are several droplets of water within the silicone. Enlarged siliconomas within the left axilla are present. There are also enlarged left internal mammory chain lymph nodes with silicone present.

No abnormality within the glandular tissue is seen on these non-contrast images.

Case Discussion

This cases demonstrates many of the features of an intra-capsular rupture of the a breast implant. MRI is the gold standard test for implant ruptures. Silicone can be present in the axillary or internal mammory lymph nodes in the presence of an intra- or extracapsular rupture and prior implant exchange.
