Prominent transmediastinal artery - testis

Case contributed by Mostafa Elfeky

Presentation

Decreased libido.

Patient Data

Age: 60 years
Gender: Male

There is a hypoechoic band-like zone obliquely crossing the upper pole of right testis. On Doppler study, avid vascularity is noted with biphasic arterial waveform consistent with a crossing arterial channel.

Case Discussion

A crossing arterial channel through testicular parenchyma is likely a prominent transmediastinal artery. Crossing vascular structures are one of the causes of striated appearance of the testis.

Differential diagnoses for this appearance include intratesticular varicocele and testicular arteriovenous malformation.
