Toxic multinodular goiter

Presentation

The patient has a known diagnosis of toxic multinodular goiter and received a right hemi-thyroidectomy in the past. He presented with a cough and a hoarse voice.

Patient Data

Age: 75 years
Gender: Male

Thyroid

The left lobe of the thyroid is generally enlarged measuring 57 x 57 x 79 mm.

There is mildly increased vascularity within the left lobe of the thyroid gland. There is a large heterogenous nodule within the left lower lobe measuring 10mm in diameter.

Technetium pertechnetate scan

The apperance of the thyroid gland is not typical given the past history of a right hemithyroidectomy. This results in hypertrophy of the isthmus and lobe of the gland.

There are 3 main focal areas of increased uptake (in a heterogenous pattern) within the isthmus and left lobe of the thyroid gland, overall findings consistent with toxic multinodular goiter.

Case Discussion

This case highlights the importance of obtaining a thorough clinical history. The technetium pertechnetate thyroid scan's appearance is atypical, given that only the left lobe of the thyroid is visualized and that there is heterogeneously increased uptake.

This patient received a hemithyroidectomy before 2000 for toxic multinodular goiter.
