Traumatic abdominal wall hernia

Presentation

Restrained driver in motor vehicle accident with prolonged extrication. Hypotensive and obtunded. Intubated on scene.

Patient Data

Age: 30 years
Gender: Male

Traumatic separation separation of all of the right oblique muscles from the right iliac bone.

Small hematoma lateral to the surface of the left external oblique muscle extending to approximately the level of the left 11th rib, extending to the area of the left iliac crest.

Moderate hematoma within the soft tissues of the thoracolumbar spine.

Case Discussion

This is a case of a traumatic abdominal wall hernia, specifically at the level of the right oblique muscles.

The patient experienced numerous additional traumatic injuries, including intracranial hemorrhage, bilateral femoral shaft fractures, and a left open ilium fracture. Orthopedic surgical stabilization consisted of bilateral femoral intramedullary nails, closed reduction, and percutaneous fixation of the left femoral neck. Following a prolonged hospitalization, the patient was discharged to rehab at the recommendation of the physical medicine and rehabilitation (PM&R) service.
