Presentation

Right upper quadrant pain for 2 days.

Patient Data

Age: 45 years
Gender: Female

A definable, area of fat stranding is noted involving right hemi-abdomen omentum adjacent to hepatic flexure and transverse colon.

Major vessels are unremarkable.

The rest of the abdominal viscera appear normal.

The intra-opearative image shows infarcted segment of the omentum in right hemi-abdomen.

Case Discussion

CT findings with clinical presentation are most likely suggestive of omental infarction as described which was surgically proven.

Omental infarction is a rare cause of acute abdomen resulting from vascular compromise of the greater omentum. This condition has a non-specific clinical presentation and is usually managed conservatively.

The classic location of primary omental infarction is in the right lower quadrant medial to the ascending colon or cecum.
