Peritoneal inclusion cyst

Presentation

A patient presented with pelvic pain. She has previous history of treatment for STI.

Patient Data

Age: 25 years
Gender: Female

Cystic lesion at the left adnexa with irregular shape which has entrapped left ovary. The cyst is inline with peritoneal surface and no discernable wall of the cyst seen.

Case Discussion

Peritoneal inclusion cysts occur as a result of the failure of the peritoneum to absorb secretions from the ovary. This usually occurs secondary to trauma, PID, surgery, or other insults to the peritoneum. Ultrasound findings include irregularly shaped cystic lesions with or without septations and without a discernable wall. These cysts can engulf adjacent organs like the ovary, resulting in an entrapped ovary appearance, as seen in our case 1,2. Such complex ovarian lesions have a long list of differential diagnoses. However, with clinical correlation of these imaging findings, a confident diagnosis can be made, thus avoiding unnecessary surgery. A correct diagnosis allows for conservative treatment, avoiding unnecessary surgery 1.
