Where is the mass?
Because hilar vessels are visible (hilum overlay sign) this means the mass is either anterior or posterior to the hilum.
What study would you recommend next?
Lateral projection (of course CT Chest will need to be done at some time for staging).
Widening and abnormal contour of the right cardiomediastinal contour including opacity projecting over the right hilum - hilar vessels are visible through the mass. No pleural effusion or pneumothorax. Right apical pleural capping. Lungs elsewhere clear. Cardiac silhouette is not enlarged. No destructive bone lesion.
