Presentation

Two large abdominal swelling in either side of abdominal midline with occasional pain, history of previous hernia repair twice

Patient Data

Age: 50 years
Gender: Female

Sizable anterior abdominal wall hernia is seen with defect measuring about 10 x 8 cm. The hernial sac is complex composed of three compartments:

  1. the first is seen to the right of the midline, neck measuring about 3.7 x 4.8 cm, containing small intestinal bowel loops. No bowel dilatation or free fluid within the sac

  2. the second is seen to the left of the midline, neck measuring about 3.6 x 4.0 cm, containing small intestinal bowel loops. No bowel dilatation or free fluid within the sac

  3. the third is almost in the midline, with the neck measuring about 5 x 5 cm, containing small part of the traverse colon and omental fat. No free fluid inside the sac
