Acetabular labral tear and paralabral cysts

Presentation

Long standing left hip pain

Patient Data

Age: 65 years
Gender: Female

Non-arthrographic MRI images demonstrate a left anterior acetabular labral tear with an associated paralabral cyst (18mm) and a right acetabular labral tear with a smaller paralabral cyst (6mm).

Mild uncovering of the femoral head relative to the acetabular cup on coronal images, suggestive of dysplastic hip morphology.

Case Discussion

Paralabral cysts of the hip joint are associated with labral tears and can be caused by an underlying developmental hip dysplasia, as is probably the cause in this case of bilateral paralabral cysts with labral tear. The presence of paralabral cyst in non-arthographic MRI, should prompt thorough search for an associated labral tear as it could look normal.
