Presentation

Right lower abdominal pain with tenderness, fever and leukocytosis.

Patient Data

Age: 20 years
Gender: Male

Dilated fluid-filled appendix (18 cm) with a thick enhanced wall, containing two appendicoliths (17 and 5 mm), obstructing the ostium with no evidence of wall defect. Periappendiceal fat stranding with thickening of the peritoneal reflections and free intraperitoneal fluid in the pelvis.

Enlarged liver with tongue-like appearance of the right lobe with inferior projection beyond the level of the most inferior costal cartilage in keeping with a Riedel lobe.

Moderately enlarged spleen (13.5 cm).

Motion artifact is noted in the upper abdomen.
