Patient Data

Age: 70 years
Gender: Male

The study reveals acute infarcts in bilateral anterior fornix showing restricted diffusion on DW scans.

Gliotic areas are seen in the right parietooccipital lobe.

Chronic lacunar infarcts and areas of ischemic demyelination are seen in

bilateral periventricular, deep, and subcortical white matter.

Convexity sulci and lateral ventricles are prominent – age-related atrophy.

Case Discussion

Acute infarcts in bilateral anterior fornix are caused by occlusion of perforating branches of the anterior communicating artery or subcallosal branches of the anterior cerebral artery. This is the cause of patient's symptoms.
