Presentation

Low back pain.

Patient Data

Age: 45 years
Gender: Male
mri

Focal herniation of L4-5 disc material is noted involving the upper endplate of L5, in keeping with Schmorl node. It is surrounded by T1 low signal and T2 high signal endplate change at represents bone marrow edema.

L3-4 and L4-5 disc desiccation, showing decreased height and hydration signals.

L4-5 diffuse posterior disc bulge.

Lumbosacral transitional vertebra with pseudoarticulation of left transverse process of L5 with left ala of the sacrum, Castellvi type 2a.

Case Discussion

Bone marrow edema surrounding intravertebral disc herniation favors acute Schmorl's node.
