Adductor longus tear

Presentation

Fall on the ground resulting in forceful right hip abduction two weeks before the presentation. Complaining of mild pain in the right groin.

Patient Data

Age: 60 years
Gender: Male
x-ray

There is no sign of acute bone injury.

ultrasound

There is a fluid-filled defect replacing the adductor longus tendon insertion location. Its tendon shows 15 mm retraction. The myotendinous junction of the adductor longus is normal. There are few tiny fluid clefts with small areas of loss of normal muscle echopattern in the adjacent pectineus muscle. The adductor brevis, adductor magnus, gracilis show normal echopattern. 

The contralateral adductors were examined for a comparison, which were normal.

Case Discussion

The case shows a complete tear of the adductor longus tendon from its insertion. Adductor injuries are commonly sports-related, unlike this case.
