Presentation

Dysmenorrhea.

Patient Data

Age: 30 years
Gender: Female

Bulky enlarged uterus with heterogeneous myometrium with multiple echogenic foci and linear shadow striations/parallel shadowing. Small subendometrial cysts  are also noted. On color Doppler, there is increased number of tortuous vessels penetrating myometrium.

CT Abdomen and pelvis

Patient has had CT to investigate abdominal pain/appendicitis, which demonstrates a bulky uterus in keeping with the previous pelvic US. Trace of free fluid in the peritoneal cavity is within the normal range for the patent's age group. No acute abdomen.

Please note that US remains the fist line for female pelvic imaging assessment and, in this case, CT was performed months later in a presentation suspicious for acute abdomen.
